Recognized as a leader in skilled rehabilitation, Lakewood’s skilled nursing and rehabilitation center provides physical, occupational and speech therapists, dedicated to helping residents achieve their highest level of independence. Collaborating with physicians and utilizing the latest equipment and techniques within a supportive atmosphere, Lakewood implements comprehensive, multi-tiered and customized skilled rehabilitation programs tailored to each resident’s specific needs and goals. Lakewood’s ultimate goal is to safely return residents to their homes, or other appropriate living arrangements, as soon as possible. While each resident’s therapy regimen is prescribed by their physicians, progress towards goals is assessed weekly in order to monitor the treatment plan to ensure clinical needs are met and/or exceeded. Once rehabilitation goals have been achieved, discharge planning occurs including home assessments and caregiver training to support a resident’s continued success in their own environment.
