Program of All-Inclusive Care for the Elderly (PACE) Midland Care’s Program of All-inclusive Care for the Elderly (PACE) is a health care program that helps people at risk for needing nursing home stay in their own homes and communities. PACE includes a team of experienced doctors, nurses and other health care professionals getting to know the participant and working with him or her to design a plan to aid in achieving health goals. PACE may be for you if you are interested in a team committed to helping you: Be as healthy as possible for as long as possible Remain in your own home and community with your family Focus on what you can do, instead of what you cannot do PACE includes health services: doctor appointments hospital visits behavioral health services dental and vision care pharmacy medical transportation day center care home support nutrition services medical supplies nursing facility care when required Participating in PACE simplifies access to health care and prevents fragmented health care that can be unsafe.
