Providence Connections is a specialized palliative care consultation program operated by Providence Health & Services in Portland, Oregon. It was established in 1994 by the Sisters of Divine Providence and has since become a vital part of Providence’s mission to provide holistic, compassionate care for individuals with serious or life-limiting illnesses. 🌿 Mission and Philosophy Providence Connections was created to enhance the quality of life for patients with complex, chronic, or terminal illnesses. Grounded in the Catholic health tradition, it operates on the principles of dignity, compassion, integrity, justice, and excellence, ensuring care for the whole person—physically, emotionally, and spiritually. 🩺 Scope of Services Providence Connections focuses on palliative care consultations and does not provide direct hospice services. Its services are integrated into hospital and outpatient settings and include: Symptom management (pain, fatigue, nausea, anxiety, etc.) Advance care planning (goals-of-care conversations, POLST forms, end-of-life decisions) Support for navigating complex medical conditions (cancer, heart failure, COPD, dementia, ALS, etc.) Coordination with primary care and specialty physicians Emotional and spiritual support for patients and families Social work and case management to support patients in home, hospital, or assisted living environments The team includes board-certified palliative care physicians, nurse practitioners, registered nurses, social workers, and chaplains. Their interdisciplinary model ensures that patients’ physical, emotional, and spiritual needs are all addressed.
